📋 Description
• Assess transitional care needs • Coordinate services across the care continuum • Support care progression • Help patients transition safely from the hospital • Partner with physicians, utilization review nurses, interdisciplinary care teams, patients, and families • Address care needs and remove barriers to discharge • Interview directly with hiring leaders for RN Care Coordinator opportunities during the virtual hiring event
🎯 Requirements
• Active Registered Nurse (RN) license • At least one year of nursing experience in an acute care setting • Current Basic Life Support (BLS) certification • Must work in the United States of America
