📋 Description

• Provide remote triage via phone or virtual platforms using evidence-based clinical protocols • Assess members’ symptoms and determine the appropriate level of care, including home care instructions, urgent escalation, or emergency response • Document encounters accurately and promptly in the electronic health record • Communicate with on-call providers, interdisciplinary teams, or emergency services as needed • Support and educate members and families to promote safety and avoid preventable emergency department visits • Manage after-hours issues related to member needs, care coordination, and service interruptions • Maintain awareness of organizational policies and escalation workflows for urgent situations • Participate in handoff communication to daytime clinical teams to ensure continuity of care • Perform additional duties and call coverage to support 24/7 operations • Perform other job-related duties as requested

🎯 Requirements

• Associate degree required • Two (2) years experience working in an acute care setting required • Two (2) years experience working in home care and MLTC/MAP required • Must be able to read, write, and speak English • Current, unrestricted Registered Nurse licensure in the state of New York required • Knowledge of local, state, and federal healthcare laws and regulations • Knowledge of Medicare and Medicaid regulations • Case Management Certification preferred • Must be able to respond promptly to escalations during assigned on-call shifts • Requires use of standard office equipment such as a computer, headset, and phone systems
