📋 Description

• Prepare comprehensive Annual Wellness Visit charts to support provider efficiency and CMS compliance • Review and summarize medical, surgical, and family histories with patients during face-to-face virtual visits via Zoom • Review medication lists and document adherence • Review preventive screenings and immunizations • Conduct cognitive, functional, and depression assessments • Identify overdue screenings, risk factors, and care gaps before visits • Communicate concise patient summaries and key findings to providers before each AWV • Coordinate with patients to obtain missing or clarified information • Ensure documentation meets Medicare, CMS, and organizational requirements • Maintain accurate, timely, and confidential patient records • Use CT360, RXNT, Zoom, electronic health record systems, and telehealth platforms • Measure and support outcomes related to clinical quality, patient engagement, and satisfaction • Adhere to HIPAA, CMS, and organizational compliance standards • Participate in professional development and ongoing training • Make independent decisions, prioritize, multitask, and perform other duties as assigned • Work 40 hours per week, Monday–Friday 9:30 AM–6:00 PM EST, one Saturday per month 10:00 AM–4:00 PM EST, with rotating holidays

🎯 Requirements

• Active, unrestricted RN or LPN/LVN license in a compact state • 3+ years of clinical nursing experience (bedside, Primary Care, Med/Surg, or Home Health) • Proven experience providing high-volume patient care in a fast-paced virtual or telehealth clinic environment • Proficient with EHR systems and telehealth/teleconferencing platforms • Dedicated, private home workspace suitable for confidential patient interactions • Strong communication skills and demonstrated ability to work independently in a remote environment • Employees must perform all work physically within the United States; international work is not permitted during scheduled work hours • Windows or Apple (macOS) computer only; no Chromebooks, Linux machines, or smartphones • At least Windows 10 or macOS 14 Sonoma • Intel Core i5 or AMD equivalent (Windows), or Apple Silicon (Macs) • 16 GB RAM • Wired headphones required; no built-in microphones • Minimum internet speed of 100 Mbps download and 50 Mbps upload • Ethernet cable connecting the computer directly to the router • Google Chrome with Amazon Workspaces • Webcam required for video calls • Experience with Medicare Annual Wellness Visits (AWVs) preferred • Knowledge of CMS documentation and preventive care guidelines preferred • Experience working across multiple virtual care programs and platforms preferred
