📋 Description

• Provide triage, case management, and care navigation for oncology patients with possible comorbidities • Deliver direct patient care through phone, email, video, and virtual platforms • Provide consultative clinical support and respond to urgent clinical escalations • Triage Symptom Survey responses and inbound reported symptoms, supporting patients or escalating to a Nurse Practitioner • Identify and address care coordination, nutritional support, discharge planning, and provider referral needs • Educate patients about their diagnosis, Daymark Health’s care offerings, and next steps • Build trusting relationships with patients and their care networks • Coordinate integrated care with Nurse Practitioners, Medical Directors, Social Workers, and non-clinical Health Partners • Address social determinants of health, including food security and transportation access • Follow policies, procedures, documentation standards, clinical protocols, and operational processes • Collaborate with Clinical Operations to optimize systems, tools, and technology

🎯 Requirements

• Bachelor of Science Degree in Nursing required • Compact unrestricted registered nurse (RN) license, or ability to obtain • Licensure in Rhode Island, or willingness to become licensed in Rhode Island • At least 3 years of nursing experience, including 2 years in oncology nursing • Passion for helping patients understand the benefits of palliative care • Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM strongly preferred, or obtain within 2 years of hire • Comfort using Slack, Zoom, Google Suite, and EHR systems • Ability to establish rapport and work effectively with diverse populations • Ability to prioritize urgent and important needs independently • Patient-centric approach and commitment to Daymark’s mission • Strong organizational, multitasking, listening, and communication skills • Adaptability in dynamic and ambiguous environments • Basic Life Support (BLS) certification required for patient-facing roles by start date • Compliance with Daymark’s Employee Vaccination and Testing policy
