📋 Description

• Oversee care coordination, clinical escalations, case management, and client placement across the CCS network • Lead specialized care and clinical case management teams in alignment with strategic objectives • Develop and implement clinical strategies to improve care path adherence, client outcomes, and provider engagement • Provide clinical oversight for high-risk cases and escalations • Oversee comprehensive assessment, planning, coordination, and evaluation of client care • Establish protocols for case reviews, risk stratification, and multidisciplinary collaboration • Monitor case management metrics and outcomes and improve service delivery and client satisfaction • Oversee Care Coordinator operations and timely processing of clinical reports and authorizations • Manage client placement processes, referral pathways, service availability, and equitable access to services • Collaborate with provider partners to optimize capacity and reduce barriers to care • Analyze clinical trends and performance data for decision-making and strategic planning • Ensure compliance with regulatory requirements and internal policies • Build relationships with provider partners and provide clinical consultation • Support professional development and training for clinical staff • Lead staffing decisions, including hiring, performance management, and team assignments • Implement strategies to prevent and manage clinical escalations and oversee complaints and grievances • Partner with Clinical Excellence and other departments on treatment plan guidelines • Represent CCS in regulatory inspections, stakeholder meetings, and cross-functional initiatives • Support care path modifications and new modalities • Complete additional responsibilities assigned by the Vice President of Network Operations

🎯 Requirements

• 8+ years of experience working in a home-health setting with supervisory responsibilities • Licensed Clinical Psychologist, LMFT, LPCC, or LCSW required • 5+ years of progressive leadership experience in clinical operations, including access and client care management oversight • Experience with individuals with autism spectrum disorders and related developmental disabilities • Proven success in strategic planning, program development, and managing remote teams • Experience in strategic planning and collaboration with executive and key operational groups preferred • Effective workplace communication, reading, and writing • Ability to work continuously with clients, staff, and others using visual and auditory abilities • Ability to use a computer, iPhone, and virtual meeting software such as Zoom and Teams • Knowledge of evidence-based methodologies including ABA, ESDM, TEACCH, PECS, and PRT • Training in ABA/Behavioral Health Treatment and knowledge of community resources and agencies serving children • Experience interpreting and implementing CCS policies, procedures, and regulations • High proficiency in Microsoft Office Suite, especially Excel and PowerPoint • Strong analytical, interpersonal, verbal, and written communication skills • Knowledge of I/DD communities and historically excluded groups • Experience working and communicating with families • Sensitivity to ethnically, linguistically, culturally, and economically diverse populations • Ability to multitask and manage day-to-day responsibilities without supervision • Ability to work with individuals at all organizational levels • Ability to meet deadlines, exercise judgment, flexibility, patience, and discretion with confidential matters • Commitment to self-reflection, feedback, and wellbeing-oriented boundaries • Commitment to a remote schedule aligned with PST business hours • Ability to obtain and maintain criminal record clearance through the DOJ • Ability to obtain and maintain clearance through the Office of Inspector General • Must attend required training
