📋 Description

• Perform outreach to members by phone regarding care plan next steps, community or health plan resources, scheduling, and ongoing education • Support members in connecting with health plan and community resources • Apply working knowledge of assigned health plan activities and resources • Serve as front-line support for member and provider inquiries, requests, and concerns • Explain care plan procedures and protocols • Support member onboarding and day-to-day administrative duties • Send welcome letters, correspondence, and program educational materials • Document and maintain non-clinical member records • Provide records to providers as needed • Perform other duties as assigned • Comply with all policies and standards

🎯 Requirements

• Must be authorized to work in the U.S. without the need for employment-based visa sponsorship now or in the future • Qualified candidates must reside in North Carolina • High School diploma or GED • 1–2 years of related experience • Knowledge of existing benefits and resources locally • Ability to make referrals to address Social Determinants of Health (SDOH) needs • Ability to comply with all policies and standards • Ability to document and maintain non-clinical member records in accordance with state and regulatory requirements
