• Develop, assess, and coordinate holistic care management activities for quality, cost-effective healthcare outcomes • Develop or assist with personalized service care plans for long-term care members • Educate members and families/caregivers on available services, benefits, procedures, provider instructions, referrals, and healthcare benefits • Evaluate member needs and available resources and recommend or facilitate plans for the best outcome • Identify providers, specialists, and community resources needed for long-term care • Coordinate members and/or families/caregivers with care provider teams to ensure services are accessible • Provide resource support for employment, housing, participant direction, independent living, justice, foster care, and other needs • Monitor care plans, member status, and outcomes and recommend updates based on member needs • Interact with long-term care healthcare providers and partners • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform home and/or other site visits as needed • Provide feedback to leadership on quality-of-care and service-delivery improvements • Perform other duties as assigned and comply with all policies and standards
• Must reside in Iredell, Rowan, Alexander, Davie, Davidson, Yadkin, Catawba, Wilkes, Lincoln, or Stanley County, North Carolina • RN or LCSW required • Bachelor's degree required, with specified qualifying-experience alternatives • 2–4 years of related experience • For North Carolina Standard Plan: two years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience • Prior experience with social work, geriatrics, gerontology, pediatrics, or human services • Experience interviewing and assessing member needs • Knowledge and experience regarding caseload management and casework practices • Knowledge regarding determining eligibility for DHSS programs • Knowledge regarding Federal and State law as it applies to DHSS programs • Ability to effectively solve problems and locate community resources • Ability to collaborate with caregivers, involved State agency representatives, and providers • Good interpersonal skills • Ability to practice Cultural Competency with awareness and respect for diversity • Knowledge of the needs and service delivery system for all populations in the case manager’s caseload • Technological savvy • Excellent communication and customer service skills • Assessment skills • Change management • LTSS and/or HCBS coordination
