• Manage overall care management and quality of care for participants • Review field staff assessments and discipline-specific documentation for quality and address deficiencies • Participate in care planning meetings and act as a member of the interdisciplinary team (IDT) • Coordinate holistic, member-centered care plans and continuum of care • Arrange, coordinate, and authorize appropriate community-based, long-term care, skilled, and support services • Coach, train, and conduct competency sessions with field staff • Monitor field staff documentation, interventions, services, utilization, and member responses • Collaborate with physicians, IDT members, facilities, discharge planners, home care coordinators, and specialty services • Document services according to CLHC standards and federal/state regulations • Evaluate care plan effectiveness and modify plans based on changes in members’ health and goals • Maintain current knowledge of health issues, procedures, evidence-based clinical guidelines, medications, and practice standards • Recommend improvements to services, programs, policies, and procedures • Follow disciplinary policies and engage Human Resources as needed • Work across sites and travel between locations as required • Perform other duties as assigned
• Graduated from a nurse practitioner education program acceptable to New York State Education Department (NYSED) • NP Board Certified • Minimum of two (2) years of Care Planning Experience • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred • Customer Service experience required • Managed long-term care insurance experience beneficial • Minimum of one (1) year of experience working with a frail or elderly population, or appropriate PACE organization training upon hiring if under one year • Supervisory experience preferred • Current and unrestricted license and registration in New York State required • Legally authorized to practice in the state where the function is performed • Medically cleared for communicable diseases and all immunizations up-to-date before direct participant contact • Bilingual preferred • Ability to meet the listed physical, cognitive, visual, hearing, mobility, lifting, and fine motor requirements • Ability to work 8:00AM–5:00PM, 40 hours weekly • Ability to work within a 2-hour commute range of the remote location
