📋 Description

• Support complex patients in navigating the healthcare system • Deliver holistic, integrated care to improve kidney patients’ quality of life • Work autonomously and collaboratively with the healthcare team • Coordinate and facilitate quality, consistent, cost-effective care • Complete comprehensive health assessments • Manage complex patients with a focus on reducing hospitalizations and improving health outcomes • Cover PST and MST time zones • Accommodate patient and dialysis center availability on a flexible Monday–Friday schedule • Provide comprehensive care and complex chronic condition management to patients nationwide • Manage patients across early kidney disease, dialysis transition, kidney transplant, and life-sustaining dialysis stages

🎯 Requirements

• Passion for delivering care to patients with complex, chronic illnesses • Minimum of two (2) years' experience as a Nurse Practitioner (NP) • Current unrestricted NP license to practice in the state • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification • Intermediate computer skills and proficiency in MS Word, Excel & Outlook required • Home Office and High Speed Internet • Willingness to obtain up to 6 additional state licensures for remote management of patient population • Spanish-speaking preferred in some markets • Preferred experience working in Value Based Care or Population Health • Experience documenting and coding from a MA (Medicare Advantage) risk perspective • Experience conducting Health Risk Assessments on high risk/chronically ill patients • 5+ years’ experience as a Nurse Practitioner practicing in adult/gerontology populations preferred • Experience with longitudinal management of chronic illness (CHF, DM, CV disease, HTN)
