📋 Description

• Support complex patients in navigating the healthcare system • Deliver holistic, integrated care to improve quality of life for kidney patients • Work autonomously and collaborate with all members of the healthcare team • Coordinate and facilitate quality, consistent, cost-effective care • Complete comprehensive health assessments • Manage complex patients with a focus on reducing hospitalizations and improving health outcomes • Cover PST and MST time zones • Work a Monday–Friday schedule accommodating patient and dialysis center availability • Maintain a flexible, full-time self-managed schedule • Provide integrated care and complex chronic condition management to patients nationwide across stages of kidney disease, dialysis, and transplantation

🎯 Requirements

• Passion for delivering care to patients with complex, chronic illnesses • Minimum of two (2) years' experience as a Nurse Practitioner (NP) • Current unrestricted NP license to practice in the state • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification • Intermediate computer skills and proficiency in MS Word, Excel & Outlook required • Home Office and High Speed Internet • Willingness to obtain up to 6 additional state licensures for remote management of patient population • Preferred experience working in Value Based Care or Population Health • Experience documenting and coding from a Medicare Advantage risk perspective • Experience conducting Health Risk Assessments on high risk/chronically ill patients • 5+ years’ experience as a Nurse Practitioner practicing in adult/gerontology populations (preferred) • Spanish-speaking preferred in some markets • Experience with longitudinal management of chronic illness, including CHF, DM, CV disease, and HTN
