📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical history during Video Visits • Educate members about their health and answer their questions • Help close gaps in care and connect members to appropriate care • Work in a private space with an adequate internet connection • Maintain training, affiliation, and compliance with Video Visit requirements • Maintain visit completion rates and capacity targets • Attend regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Provide Video Visit telehealth services only • Operate computers, phones, iPads, and other electronic devices while interacting remotely with members

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Board certification required • Active state licenses in coverage areas • Ability to obtain and maintain multi-state licenses in coverage area, as required • FNP or AGNP certification preferred for NPs • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Availability for 40 hours weekly • Consistently provide 320+ capacity units per month • Consistently complete 220+ visits per month • Training and credentialing for all health plan clients and lines of business in primary coverage state(s) • Regular participation in manager, team, and clinical meetings • Broadband connectivity with video and audio enabled • Quiet setting with a neutral background for conducting visits • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology
