• Responsible for overall care management and quality of care for participants. • Review assessments and discipline-specific documentation for quality and address deficiencies with field staff. • Participate in and represent the social work discipline in care planning meetings. • Monitor field-staff documentation and interventions. • Conduct coaching, competency, and training sessions with field staff as needed. • Coordinate a continuum of care consistent with members’ healthcare needs and goals. • Arrange, coordinate, and authorize appropriate member-specific services, including ADLs/IADLs assistance, housing, meals, transportation, and community-based services. • Implement care management activities and interventions to accomplish participant goals. • Collaborate with team members, facilities, discharge planners, and home care coordinators during care-setting transitions. • Document services according to CLHC standards and federal/state regulations. • Coordinate long-term care services in nursing homes, rehabilitation facilities, and other settings. • Collaborate with PCPs, specialty physicians, and the interdisciplinary team regarding participant condition changes and resource coordination. • Provide or arrange skilled services, service authorization, periodic reassessment, and evaluation. • Monitor care activities, services, member responses, plan effectiveness, and service utilization; adjust plans as needed. • Maintain current knowledge of health-related issues, procedures, evidence-based guidelines, medications, and practice standards. • Recommend and contribute to improvements in services, programs, policies, and procedures. • Follow disciplinary policies and engage Human Resources regarding disciplinary actions and terminations. • Act within the scope of authority to practice and meet organizational competencies before working independently. • Perform other duties as assigned. • Work a schedule of 8:00 AM–5:00 PM, 40 hours weekly.
• Graduated from a Master Social Work program acceptable to New York State Education Department (NYSED.) • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency (CHHA), Manage Care, long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred. • Customer Service experience required. • Managed long-term care insurance experience beneficial. • Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. • Supervisory experience preferred. • Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function. • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. • Current active and unrestricted license and registration in New York State required. • Bilingual, preferred. • Ability to meet the stated physical, visual, auditory, motor, and cognitive requirements. • Ability to travel and walk between sites/locations and work areas throughout the day.
