📋 Description

• Assess and screen patients' daily living and work-related skills and develop therapeutic retraining programs with measurable objectives • Administer and interpret diagnostic and prognostic functional tests in patients' homes or other settings • Confer with physicians and healthcare team members and participate in developing and revising Plans of Care Treatment • Provide therapeutic treatment and instruction to improve or restore strength, coordination, range of motion, and function, or teach compensation measures • Review and expand retraining programs according to each patient's behavioral goals and the overall rehabilitation program • Consult with vocational team members to develop and implement vocationally oriented plans • Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members • Participate in care coordination and discharge planning and support other healthcare personnel in meeting patient needs • Design community reintegration activities and coordinate plans with the interdisciplinary team • Recommend or design adaptive equipment to improve home or work environments and participation in rehabilitation • Train staff and family members in retraining programs, including dressing, feeding, grooming, and hygiene skills • Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance • Document patient care observations, interventions, and evaluations on the day services are rendered • Document and implement interim physician orders accurately • Submit evaluations, treatment plans, and discharge summaries to supervisors and care management staff

🎯 Requirements

• Current and unrestricted occupational therapist licensure • Current CPR certification • Valid driver's license • Auto insurance meeting residing-state minimum limits or specified Humana limits, whichever is higher • Reliable transportation • Minimum six months of occupational therapy experience preferred • Home Health experience preferred • Ability to complete TB screening if selected • Scheduled for 40 hours per week
