• Conduct hospice IPU/HCFC/LSC accreditation and/or certification surveys • Plan, organize, and coordinate community-based healthcare surveys • Execute visits according to CHAP Standards of Excellence • Comply with CHAP policies and procedures governing accreditation/certification • Maintain confidentiality except where required by law • Complete required CHAP internal and QSEP LSC orientation and applicable training before independent visits • Produce accurate, complete, and well-written documentation • Travel to hospice agencies being surveyed nationwide
• Bachelor's degree in related field required • Master's degree preferred • Current Social Work license required • Minimum of 5 years of compliance experience within the hospice industry • At least 1 year of leadership experience, including responsibility for leading and/or supervising others • Prior experience working in a healthcare facility, such as hospice inpatient units, skilled nursing facilities, healthcare facility administration, Life Safety Code compliance, or other healthcare regulatory/compliance-related roles • Strong working-level knowledge of the CMS Hospice Conditions of Participation, including the full scope of the CoPs and related resources • Knowledge of the CHAP Standards of Excellence is a plus • Strong attention to detail • Ability to deliver accurate, complete, and well-written documentation • Ability to work autonomously • Critical-thinking ability • Adaptability to change • Completion of required CHAP internal and QSEP LSC orientation and applicable training before conducting independent Hospice IPU/LSC site visits • Availability for a minimum of 5 survey days per month; travel days do not count toward survey days • Ability to travel to hospice agencies nationwide
