📋 Description

• Communicate resources and services available to patients through the continuum of care • Maintain patient chart compliance through proper documentation and updates of medical history, medication, immunizations, allergies, surgical history, and family history • Assist patients with understanding care instructions, navigating services, and addressing non-clinical questions • Answer inbound patient calls with professionalism, empathy, and clarity • Conduct outbound calls for follow-ups, appointment reminders, wellness checks, or care coordination • Maintain patient confidentiality and adhere to HIPAA standards • Follow call scripts, workflows, and escalation pathways • Meet performance metrics such as call quality, accuracy, and responsiveness • Work closely with the clinical team and dedicated nurses to ensure seamless coordination and a positive patient experience

🎯 Requirements

• Active and unencumbered certification from an accredited regulatory body such as AAMA, NHT, NHA, NCCT, Pharmacy Technician registries, and State Nurse Aide registries • Accepted certifications include Certified Nursing Assistant (CNA), Certified Clinical Medical Assistant (CCMA), Certified Pharmacy Technician (CPhT), Patient Care Technician (CPCT/A), Emergency Medical Technician (EMT), and Licensed Respiratory Therapist (RT or CRT)
