• Communicates resources and services available to patients through the continuum of care. • Maintain patient chart compliance through proper documentation and updates of: medical history, medication, immunizations, allergies, surgical history, and family history. • Assist patients with understanding care instructions, navigating services, and addressing non-clinical questions • Answer inbound patient calls with professionalism, empathy, and clarity • Conduct outbound calls for follow-ups, appointment reminders, wellness checks, or care coordination • Maintain patient confidentiality and adhere to HIPAA standards • Follow call scripts, workflows, and escalation pathways • Meet performance metrics such as call quality, accuracy, and responsiveness
• Active and unencumbered certification from an accredited regulatory body such as AAMA, NHT, NHA, NCCT, Pharmacy Technician registries, and State Nurse Aide registries. • Accepted certifications include: • Certified Nursing Assistant (CNA) • Certified Clinical Medical Assistant (CCMA) • Certified Pharmacy Technician(CPhT) • Patient Care Technician (CPCT/A) • Emergency Medical Technician (EMT) • Licensed Respiratory Therapist (RT or CRT)
