📋 Description

• Provide direction, education, leadership, and mentorship for hospital case management departments and DPOs • Lead development, implementation, and evaluation of companywide patient care programs related to case management • Develop and manage quality case management operational models according to community needs and industry standards • Develop guidelines for working with insurers, managed care organizations, referral providers, patients, and families • Evaluate census, acuity, skill level, and staff availability to manage staffing needs and caseloads • Ensure facilities adhere to state, TJC, CMS, and other accrediting standards • Ensure timely and accurate IRF-PAI submissions • Ensure FOM training is provided to hospital staff • Oversee chart review processes and documentation accuracy across hospitals • Oversee the IRF-PAI final rule audit • Monitor and evaluate Net Promoter Score (NRC) at each facility and provide education as needed • Participate in annual capital and operating budget preparation and manage department expenses • Comply with approved safety standards and perform other duties supporting organizational effectiveness

🎯 Requirements

• Five years recent experience in acute inpatient rehabilitation hospital setting preferred • Current state clinical licensure required • RN license preferred • Must maintain acceptable driving record, current driver’s license, and insurability • Knowledge of clinical operations and procedures • Knowledge of compliance and regulatory standards governing health plans and patient care • Understanding of treatment costs and financial support as they relate to quality and efficiency • General computer skills including data entry, word processing, email, and record management • Critical thinking skills • Ability to prioritize, meet deadlines, and complete complex tasks • Ability to maintain quality and safety standards • Ability to maintain confidentiality • Ability to travel between hospital locations • Ability to work closely and professionally with others at all levels of the organization • Effective organizational and time management skills • Excellent verbal and written communication skills • Ability to perform the stated physical requirements, including prolonged sitting, lifting/exerting up to 10 lbs, manual dexterity, close vision, and adjusting focus
