• Provide safe, efficient, patient-centered, evidence-based medication services • Verify medication orders and/or dispense medications accurately according to state and federal regulations • Document services completely, accurately and timely • Ensure pharmacy operations comply with national accreditation standards • Maintain professional interactions with patients • Educate and train students, residents, caregivers, and/or patients • Counsel patients and conduct follow-up calls • Provide expert drug information • Review oral and chemotherapy medication orders for appropriate dosing, frequency, and administration • Assess patient profiles and identify safe medication combinations • Work varying shifts as needed in a PRN role
• PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy • Current state licensure as a Registered Pharmacist (RPh), or eligibility for reciprocation in the current state within 90 days of hire • For individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) • For individuals required to administer immunizations: completion of a Board of Pharmacy-approved immunization administration course or certification obtained within 90 days of hire • Ability to understand drug indications, effects, side effects, and interactions • Ability to assess patient profiles and identify safe medication combinations • Ability to stand and walk for prolonged periods, approximately 75% of the shift • Manual dexterity for computer and dispensing functions • Ability to lift up to 30 lbs. • Ability to perform reaching, stretching, stooping, climbing steps, and sitting • Normal or corrected vision • Ability to communicate clearly by phone or in person • Compliance with required personal protective equipment • Compliance with annual influenza vaccination program or approved exemption • Preferred: board certification • Preferred: PGY1 residency, fellowship, or 2 years of relevant post-graduate pharmacy experience • 1–2 years of oncology experience highly preferred • PGY2 residency or relevant experience preferred for specialized pharmacy practice areas
