📋 Description

• Lead and provide operational oversight for Behavioral Health Care Managers and Associate Behavioral Health Care Managers • Train, supervise, and provide ongoing performance feedback to ABHCMs and BHCMs • Provide clinical and administrative guidance, case consultation, and support for complex cases • Ensure Collaborative Care services align with CoCM principles, including systematic case review, measurement-based care, and interdisciplinary communication • Support comprehensive assessments, brief evidence-based interventions, and care coordination with oncology teams • Monitor caseloads, workflows, program metrics, performance indicators, patient satisfaction, and care outcomes • Liaise with behavioral health providers, oncology providers, and psychiatric consultants • Ensure compliance with organizational policies, HIPAA, CMS guidelines, billing requirements, and applicable regulations • Conduct chart audits and oversee documentation practices • Contribute to clinical workflow development, resource allocation, continuous improvement, data collection, and program evaluation • Collaborate with Outreach and Partner Success to identify behavioral health needs and expand integrated care access • Maintain a limited active patient caseload • Conduct behavioral health assessments, deliver brief evidence-based interventions, and monitor treatment progress • Coordinate patient-centered treatment planning with psychiatric consultants, oncology teams, and medical providers • Serve as a clinical role model for ABHCMs and BHCMs

🎯 Requirements

• Master’s degree in Social Work, Counseling, Psychology, or related field • Active clinical license, such as LCSW, LPC, LMFT, or similar, in at least one state • Minimum 3–5 years of behavioral health clinical experience • Experience with the Collaborative Care Model or behavioral health in medical settings • Strong interpersonal, leadership, and organizational skills • Experience in oncology, palliative care, or chronic disease behavioral health preferred • Familiarity with CoCM billing, documentation, and population health management tools preferred
