• Conduct clinical assessments and diagnose within scope • Develop collaborative treatment plans using approved formats • Provide individual, group, and family interventions using evidence-based practices • Maintain timely, compliant documentation • Meet supervision, training, and licensure requirements • Contribute to culturally sensitive therapeutic programs • Participate in committees and follow organizational policies • Engage as an active PCHH team member • Support patient self-management goals, proactive care, and case reviews • Provide 1–2 open appointments each day for internal referrals within CommWell Health • Collaborate with experienced clinicians and leaders • Participate in team huddles, recognition programs, and a collaborative care model
• Graduation from an accredited/approved college or university with a master’s degree in counseling, psychology, social work, or another human-services-related field • LPA, LMFT, LCSW, LCAS, LPC, or LCMHC license, with eligibility for licensure within 24 months of date of hire • CPR/AED/First Aid Certificate from the American Red Cross or American Heart Association, or BLS Certificate and Basic First Aid Certificate from the American Red Cross or American Heart Association • NCI+ Preventive Certified • Ability to conduct clinical assessments and diagnose within scope • Ability to develop collaborative treatment plans using approved formats • Ability to provide individual, group, and family interventions using evidence-based practices and ethical and legal guidelines • Ability to maintain timely, compliant documentation • Ability to meet supervision, training, and licensure requirements
