📋 Description
• Accepts and facilitates new referrals for both in and out of the system for home health and hospice patients • Acts as a liaison to provide community education for our referral sources, patients and families • Provides accurate and responsive service to all calls received during weekend hours • Collaborates with physicians and obtains orders as needed to facilitate patient care • Provides guidance and support to field staff during hours worked • Assists in scheduling patient admissions, routine visits, and PRN visits to field staff (Nurses, Therapists, and Home Health Aides) • Monitors telehealth as needed • Performs other duties as assigned
🎯 Requirements
• Associate or Bachelor's degree in Nursing required • Two (2) years of experience as a registered nurse in a clinical care setting or home health preferred • Active and unencumbered Multi-State Registered Nurse license in state(s) of employment required • Current CPR certification required
