📋 Description
• Triage all patient and facility calls after hours. • Facilitate new and existing home health referrals from in and out of system. • Provide accurate and responsive service to all calls received during weekend hours. • Collaborate with physicians and obtain orders as needed to facilitate patient care. • Provide guidance and support to field staff during hours worked. • Assist in scheduling patient admissions, routine visits, and PRN visits to field staff.
🎯 Requirements
• Associate or Bachelor's degree in Nursing required. • Two (2) years of experience as a registered nurse in a clinical care setting or home health preferred. • Active and unencumbered Multi-State Registered Nurse license in state(s) of employment required. • Alaska, California, and Michigan RN licensure required. • Current CPR certification required.
