📋 Description
• Responsible for programmatic and strategic oversight of Pharmacy Clinical Programs • Collaborates closely with pharmacy leaders to enhance patient outcomes • Plans and executes new clinical programs • Standardizes medication utilization across the System • Leads Formulary and Pharmacy and Therapeutics committees • Develops and implements medication therapy policies
🎯 Requirements
• Doctor of Pharmacy Degree (Pharm.D.) • 6 years of experience planning and executing pharmacy programs • 10 years of experience in a hospital setting • Current Pharmacy license • Board Certified Oncology Pharmacist (BCOP) • ASHP accredited PGY-1 or PGY-1 and PGY-2 Residencies preferred • 5 years of experience in Oncology preferred
