📋 Description

• Provide guidance, support, consultation and referrals to covered employees and dependents • Conduct comprehensive clinical assessments, including identification of presenting issues, work-life challenges, and screening for risk and safety concerns • Develop individualized care plans addressing each participant's unique clinical and sub-clinical needs • Facilitate referrals and assist participants with securing appointments for short-term EAP counseling • Provide mental health advocacy and navigation for participants needing higher levels of care • Assist with referrals to other employer-sponsored health management resources when appropriate • Conduct active case management, including treatment plan reviews • Perform timely follow-up to ensure connection to care, program satisfaction, and determine the outcome and impact of EAP services • Maintain accurate and complete records in compliance with HIPAA, ethical and departmental guidelines • Provide in-the-moment telephonic and digital support and personalized guidance to program participants

🎯 Requirements

• Master's Degree in a Mental Health Discipline (e.g. - Social Work, Counseling, Marriage and Family Therapy) • Relevant Post-Graduate Clinical Experience • Clinical Licensure • Familiarity and Comfort with Technology to Train and Work Remotely (e.g. - Microsoft Office Suite, Contact Management Systems, Web-Based Software, Soft Phone) • Strong Reasoning Ability – Capacity to Collect Data, Establish Facts, Define Problems and Draw Valid Conclusions • Ability to Thrive in a Fast-Paced, Fully Remote Work Environment • Prior EAP and/or Case Management Experience a Plus • Must be located in the continental United States or Hawaii
