📋 Description

• Provide after-hours telephonic care and clinical direction to residents and facility staff • Serve as the first line of support during evenings, nights, weekends, and holidays • Take and place calls using Curana’s telephonic platform • Coordinate care between facilities, hospitals, and clinics • Deliver care for acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers • Perform comprehensive assessments and accurately document encounters in the EMR in compliance with CMS requirements • Evaluate acute changes, falls, and controlled substance visits through telehealth • Apply clinical protocols and practice guidelines to support treatment in place whenever possible • Participate in mandatory education and training • Work two weeknight shifts from 8pm–2am • Provide every-other-weekend coverage with 12-hour night shifts on Saturday and Sunday, 6pm–6pm • Provide required overnight and holiday coverage

🎯 Requirements

• Master's Degree as a Nurse Practitioner • Active licensure in North Carolina and South Carolina required • Additional active licenses in Florida and Georgia strongly preferred • Must be willing to obtain additional state licenses as needed within a 30-day timeframe • Nurse Practitioner national certification as ANP, FNP, or GNP • Ability to obtain DEA licensure / Prescriptive Authority • Background in acute and chronic disease management • Clinical background in adult, family, or geriatrics • 3+ years of experience as a NP • Ability to gain a collaborative practice agreement, if applicable in your state(s) • Ability to work scheduled shifts in accordance with scheduling policies • Proficient computer skills, including documenting medical information with written and electronic medical records • Experience working in a nursing home or with seniors in an acute care facility preferred • Understanding of geriatrics, chronic illness, and acute disease management preferred • Understanding of advanced illness and end-of-life discussions preferred • Ability to develop and maintain positive customer relationships • Adaptability to change • Must be able to manage high call volumes and multiple calls independently • Must perform at least 30% telehealth visits • Must be available for overnight and holiday shifts, including 2 holidays per year • Visa sponsorship is unavailable
