📋 Description

• Provide after-hours telephonic direction and medical care to residents and facility staff • Serve as the first line of support during evenings, nights, weekends, and holidays • Use Curana’s telephonic platform to take and place calls • Coordinate care between facilities, hospitals, and clinics • Deliver care for acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers • Perform at least 30% telehealth visits, including evaluations of acute changes, falls, and controlled substance visits • Perform comprehensive assessments and accurately document encounters in the EMR in compliance with CMS requirements • Apply clinical protocols and practice guidelines to support treatment in place when possible • Participate in mandatory education and training • Work 24 hours per week on a set schedule, including two weeknight shifts from 8pm–2am and every-other-weekend 12-hour night shifts on Saturday and Sunday • Provide overnight and holiday coverage for skilled nursing and senior living facilities

🎯 Requirements

• Master's Degree as a Nurse Practitioner • Active licensure in North Carolina and South Carolina required • Additional active licenses in Florida and Georgia strongly preferred • Willingness to obtain additional state licenses as needed within a 30-day timeframe • Nurse Practitioner national certification as ANP, FNP, or GNP • Ability to obtain DEA licensure / Prescriptive Authority • Background in acute and chronic disease management • Clinical background in adult, family, or geriatrics • 3+ years of experience as a NP • Ability to gain a collaborative practice agreement, if applicable in applicable state(s) • Ability to work scheduled shifts in accordance with scheduling policies • Proficient computer skills, including documenting medical information with written and electronic medical records • Preferred: experience working in a nursing home or with seniors in an acute care facility • Preferred: understanding of geriatrics, chronic illness, and acute disease management • Preferred: understanding of advanced illness and end-of-life discussions • Preferred: ability to develop and maintain positive customer relationships • Preferred: adaptability to change • Must be able to manage high call volumes and multiple calls independently • Must perform at least 30% telehealth visits • Must provide overnight and holiday coverage, including two holidays per year • Company unable to provide visa sponsorship
