📋 Description

• Function as part of the National After-Hours Team and report to the NAHT Director of Clinical Operations or Clinical Team Lead • Perform activities within the RN scope of practice under provider or DCO/CTL direction • Provide telephonic and virtual after-hours care and direction to members and facility staff • Deliver medical care services to a pre-designated group of enrollees in the Midwest Region • Provide coverage during weeknights, weekends, and holidays • Perform telephonic triage of incoming calls and voicemails to determine the appropriate level of care • Collaborate with nursing facilities, APPs, and community members regarding changes in condition • Support inbound and outbound member communications, handoffs, telephonic assessments, and acuity follow-ups • Coordinate follow-up for members in the ER • Participate in onboarding and orientation activities for new clinical staff under DCO/CTL direction • Document clinical information, telephonic assessments, and activities in the appropriate documentation tool • Complete RN-to-APP handoffs and escalate appropriate calls to the NAHT APP • Use RN mini SOAP notes for acute-change calls and fall mini SOAP notes for reported falls

🎯 Requirements

• Current, unrestricted compact RN license in primary state • Additional licensure in assigned states must be obtained within 6 months of hire if home state does not participate in compact license • 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management position • Dedicated space for home office setup • Access to high-speed internet services • Computer/typing proficiency to enter and retrieve data in electronic clinical records • Proficient with Microsoft Word, Outlook, and Excel • Strong problem-solving skills • Ability to communicate complex or technical information understandably and interpret complex information from others • Ability to adapt to change and perform positively and efficiently in a production-driven environment • Overnight and holiday availability required • Company unable to provide visa sponsorship
