📋 Description

• Conduct telephonic outreach to and enroll eligible Medicaid members • Engage members using caring and behavioral interviewing techniques • Support comprehensive coordination of healthcare services • Facilitate case assignment • Connect identified members to care managers through warm transfers • Support implementation of care plans and effective healthcare-service utilization • Promote and support quality effectiveness • Answer telephone calls, take messages, research information and assist in solving problems for team members • Complete accurate and complete documentation meeting risk management, regulatory and accreditation requirements • Protect member confidentiality and follow company confidentiality policies • Meet Member Connection Team efficiency, productivity and daily performance metrics

🎯 Requirements

• 1-2+ years strong customer service skills to coordinate service delivery • Attention to customers, sensitivity to issues, proactive identification and resolution of issues • Adherence to care management processes, privacy and confidentiality, quality management processes, regulatory and accreditation guidelines, and company policies and procedures • Ability to complete detailed electronic documentation of each member call • Ability to work independently and meet deliverables and deadlines • Ability to effectively participate in a multi-disciplinary team • Effective communication, telephonic and organizational skills • Ability to manage multiple priorities and adapt to change • Ability to meet daily metrics with speed, accuracy and a positive attitude • HS Diploma required • 2-4 years’ experience in a call center healthcare field preferred • Experience with Microsoft Word, Outlook, and Excel, data entry and member-record documentation preferred • Familiarity with basic medical terminology and care management concepts preferred • Flexibility to work occasional nights and weekends outside standard business hours, which can span from 8:00 am to 8:00 pm • Strong verbal and written communication skills preferred • Bilingual Spanish preferred • LA Medicaid experience preferred
