📋 Description

• Conduct telephonic outreach to and enroll eligible Medicaid members • Support comprehensive coordination of healthcare services • Engage members using caring and behavioral interviewing techniques • Conduct direct member outreach and engagement • Facilitate case assignment and connect identified members to care managers through warm transfers • Support implementation of care plans to promote effective utilization of healthcare services • Provide support to team members by answering telephone calls, taking messages, researching information, and assisting in solving problems • Comply with policies, procedures, and regulatory standards • Maintain accurate and complete documentation meeting risk management, regulatory, and accreditation requirements • Protect the confidentiality of member information

🎯 Requirements

• Strong communication, organizational, and customer service skills • Ability to manage multiple priorities and adapt to change • Ability to consistently meet productivity and quality metrics with accuracy and a positive attitude • Strong attention to detail and documentation skills • Ability to work independently while collaborating effectively with team members • Professional, enthusiastic, and member-focused approach to outreach • HS Diploma or GED equivalent • Monday through Friday availability, 8am-5pm Central Time Zone • 40 hours per week
