📋 Description

• Conduct telephonic and occasional in-person assessments to understand member needs • Develop and manage personalized care plans to improve health and wellness • Coordinate care across programs, providers, and resources • Identify risks and remove barriers to care using clinical judgment • Engage members through meaningful conversations to support health goals • Collaborate with a multidisciplinary team to optimize outcomes

🎯 Requirements

• Active, unrestricted multistate RN license in state of residence • 3+ years of clinical nursing experience • Strong computer skills, including Microsoft Office and web-based systems • Ability to work Monday–Friday, 8:00am–5:00pm CST • Occasional rotation to 10:00am–7:00pm CST • Associates Degree in Nursing required • Case management experience in an integrated model preferred • Bilingual Spanish/English preferred • Bachelor's in Science & Nursing preferred
