📋 Description

• Conduct telephonic and occasional in-person assessments to understand member needs • Develop and manage personalized care plans to improve health and wellness • Coordinate care across programs, providers, and resources • Identify risks and remove barriers to care using clinical judgment • Engage members through meaningful conversations to support their health goals • Collaborate with a multidisciplinary team to optimize outcomes

🎯 Requirements

• Active, unrestricted multistate RN license in state of residence • 3+ years of clinical nursing experience • Strong computer skills (Microsoft Office and web-based systems) • Ability to work the required schedule • Associates Degree in Nursing required • Case management experience (integrated model) preferred • Bilingual (Spanish/English) preferred • Bachelor's in Science & Nursing preferred
