📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical histories • Perform physical evaluations • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Use an iPad and intuitive clinical workflow for each evaluation • Provide education and help members understand and manage their health • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Build trusted relationships and help bridge gaps in care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Able to cover the following primary locations: Otsego County, Albany NY • Able to work safely and comfortably in members’ homes, including environments with household pets, secondhand smoke and potential airborne allergens or irritants • Willingness and ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow for evaluations
