📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with members and build trusted relationships • Review medications and medical histories • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide as part of efforts to connect them to appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Work as a clinician colleague fulfilling the requirements of the specified role

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) SEATTLE SNOHOMISH • Board certification (required) • Multi-state licenses (preferred) • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Must be flexible to travel locally and within licensed states, as needed • Must be able to conduct virtual visits as needed • Must be able to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Must be able to use an iPad and clinical workflow
