📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Help identify connections to appropriate care and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Nurse Practitioner or Physician Assistant qualification • Able to cover Denver/Arapahoe County • Able to travel locally and within licensed states as needed • Able to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Able to conduct virtual visits as needed
