📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Help identify connections to appropriate care and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting • Filing of the appropriate attestation with the state of Minnesota • Board certification • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
