📋 Description

• Provide in-home health evaluations in members’ residences • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and within licensed states • Visit people nationwide and connect them to appropriate care • Provide health education and answer members’ questions

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting • Filing of the appropriate attestation with the state of Minnesota • Board certification • Multi-state licenses (preferred) • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to stay overnight in hotels and use car rentals and/or flights as needed • Ability to safely perform duties in homes with household pets, secondhand smoke, and potential airborne allergens or irritants
