📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams, spirometry, and bone density screening • Build trusted relationships and help identify connections to appropriate care • Travel locally and within licensed states across a defined coverage area • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Visit members in their private residences and work in diverse home environments

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, including up to 100% travel • Ability to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens or irritants • Ability to use an iPad and clinical workflow • Ability to conduct virtual visits as needed
