📋 Description

• Provide in-home health evaluations in members’ homes • Travel up to 100% of the time across a defined coverage area, potentially requiring overnight hotel stays, car rentals, and/or flights • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services • Visit health plan members nationwide and connect them with appropriate care • Provide health education and answer members’ questions • Work in members’ private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Availability for 40 hours per week • Willingness and ability to travel locally and within licensed states • Ability to conduct virtual visits as needed
