📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Visit people nationwide and connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, as needed • Ability to work safely and comfortably in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow for evaluations
