📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with health plan members in their homes and build trusted relationships • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Answer health questions and provide health education

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Flexibility to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Ability to use an iPad and clinical workflow for evaluations
