📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit health plan members nationwide and connect them to appropriate care • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Educate members and answer their health questions

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Ability to travel locally and within licensed states as needed • Nurse Practitioner or Physician Assistant qualification • Full-time availability, 40 anticipated weekly hours
