📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Engage with members, answer health questions, and provide health education • Help identify and bridge gaps in care • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
