📋 Description

• Provide In-Home Health Evaluations in health plan members’ homes • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Engage with members, answer health questions and provide health education • Help identify connections to appropriate care and gaps in care • Conduct virtual visits as needed • Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Flexibility to travel locally and within licensed states, as needed • Nurse Practitioner or Physician Assistant qualification
