📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Engage with members, answer health questions, and provide health education • Help identify connections to appropriate care and close gaps in care • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Provide Signify Health Diagnostic & Preventive Services products • Avoid prescribing medication, ordering lab tests, or altering current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
