📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic and preventive services when ordered, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Help identify connections to appropriate care and bridge gaps in care • Visit health plan members in their private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and intuitive clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and ordered diagnostic tests
