📋 Description

• Provide approximately 45-minute in-home health evaluations • Build trusted relationships with health plan members and help improve health outcomes • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Connect members to appropriate care and help bridge gaps in care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
