📋 Description

• Provide in-home health evaluations in members' residences • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the member's health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and intuitive clinical workflow for each evaluation • Travel locally and within licensed states across a defined coverage area • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Build trusted relationships and help bridge gaps in care • Connect members to appropriate care for their unique needs

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Able to travel locally and within licensed states • Able to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Anticipated availability of 40 hours per week
