📋 Description

• Provide in-home health evaluations in health plan members’ residences • Conduct approximately 45-minute visits involving medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Engage with members, answer health questions, and provide health education • Help identify connections to appropriate care and bridge gaps in care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening • Work as part of a national network of clinicians improving health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to work safely in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Ability to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
