📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services • Travel locally and within licensed states • Build trusted relationships and help identify appropriate care and referrals • Visit people nationwide to support improved health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Able to travel up to 100% of the time across a defined coverage area • May require overnight hotel stays, car rentals and/or flights • MD or DO qualification stated in the required and preferred qualifications
