📋 Description

• Provide In-Home Health Evaluations in members’ homes • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical histories • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states to cover assigned counties and business needs • Engage with members, answer health questions, and provide health education • Visit health plan members nationwide and connect them with appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to stay overnight in hotels and use car rentals and/or flights as required • Ability to safely perform duties in homes containing household pets, secondhand smoke, and other airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification
