📋 Description

• Provide in-home health evaluations in members’ homes • Engage with members and build trusted relationships • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and within licensed states, covering all counties and other areas of business need • Help identify connections to appropriate care and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to travel locally and within licensed states • Able to travel up to 100% of the time • Able to stay overnight in hotels and use car rentals and/or flights as needed • Able to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens and other irritants • Nurse Practitioner or Physician Assistant qualification • Anticipated availability of 40 hours per week
