📋 Description

• Provide In-Home Health Evaluations in members’ homes • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical histories • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states, covering all counties and areas of business need • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Educate members and answer health questions • Help bridge gaps in care and connect members to appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Nurse Practitioner or Physician Assistant qualification • Able to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals, and/or flights • Ability to conduct virtual visits as needed
